Excoriation
Loss of epidermis following trauma
Lichenification
Well-defined roughening of the skin with accentuation of skin markings
Scales
Flakes of stratum corneum
Crust
Rough surface consisting of dried serum, blood, bacteria & cellular debris, that has exuded through an eroded epidermis
Scar
New fibrous tissue which occurs post wound healing, may be atrophic, hypertrophic or keloidal
Koebner phenomenon
A linear eruption arising at the site of trauma
Ulcer
Loss of epidermis and dermis
Fissure
Epidermal crack through to dermis (often due to excessive dryness)
Striae
Linear areas which progress from pink to white, with the histopathalogical appearance of a scar
Macule
Flat area of altered colour <1 cm in diameter
Patch
Flat area of altered colour or texture >1cm in diameter
Papule
Small, raised lesion <1cm in diameter
Nodule
Solid, raised lesion >1cm in diameter with a deeper component
Plaque
Palpable, scaling, raised lesion >1cm in via,eter
Vesicle
Small blister
Raised, clear fluid filled lesions <1cm diameter
Bulla
Large blister >1cm in diameter
Pustule
Pus containing lesion, small and circumscribed
Abscess
Localised accumulation of pus on the dermis or subcutaneous tissues
Wheal
Transient raised lesion due to dermal oedema, demarcated but not stable borders
