Name the bulls and blistering skin diseases- most have pemphigus or “p” in the name.
1) Pemphigus Vulgaris

Causes of pemphigus vulgaris?
Idiophatic
Ace inhibitors
Penicillamine
Characteristic of P vulgarism?
Thin fragile skin that comes off easily - Nikolskly sign
Age: 30-40s
Severity: Life-threatening
Mouth involvement: yes
Diagnosis of P Vulgaris?
Skin biopsy
Tx of P vulgarism?
Steroids -> Prednisone –if doesn’t work– use Azathioprine, Mycophenolate, Cyclophosphamide.
Causes of Bullous pemphigoid?
Most often drug related -sulfa being a biggie.
Characteristics of B. pemphigoid?
Deep, thicker walled blisters. Less likely to rupture.
Age: 70-80s
Severity: Mortality is less likely – less chance of infection
Mouth involved: rarely
Dx of B. pemphigoid?
Biopsy– Immunofluorescent antibodies
Tx of B. pemphigoid?
Steroids- Prednisone
Alternative: Tetracycline or Erythromycin with nicotinamide
Causes of Pemphigous Foliaceus?
Autoimmune or drug induced–ACEi or NSAIDs
Dx and Tx
Biopsy
Steroids or Azathioprine, Mycophenolate or Cyclophosphamide
Causes of porphyria cutanea tarda
Bad porphyrin metabolism = photosensitivity reaction because high accumulation of porphyrin in the skin
Associated with PCT?
Alcoholism Liver dz Chronic hepatitis C- other liver Dzs Oral Contraception Diabetes
Characteristics of PCT
Sun exposure form non-healing blisters
Skin can become hyper pigmented
Hypertrichosis of the face– abnormal amount of hair.
Dx of PCT
Urine test – look for uroporphyrins 2-5 higher than coproporphyrins
