Mx of eczema?

A

Maintenance:
Soap substitutes
Emollients -
* Thin creams: E45, aveeno, diprobase
* Thick, greasy emollients: hydromol, diprobase, epaderm

Flares:
* Mild: Hydrocortisone 0.5%, 1% and 2.5%
* Moderate: Eumovate (clobetasone butyrate 0.05%)
* Potent: Betnovate (betamethasone 0.1%)
* Very potent: Dermovate (clobetasol propionate 0.05%)

What is eczema herpeticum?

A

A viral skin infx in pts w/eczema caused by HSV or VZV. Pts can be very unwell - derm emergency.

A

What is Stevens-Johnson syndrome (SJS) and toxic epidermal necrolysis (TEN)?

A

A disproportional immune response causes epidermal necrosis, resulting in blistering + shedding of top skin layer.

SJS - affects <10% of body SA
TEN - affects >10% of body SA

A

Medications:
* Anti-epileptics
* Abx
* Allopurinol
* NSAIDs

Infx:
* HSV
* HIV
* CMV
* Mycoplasma pneumonia

Sx of Stevens-Johnson syndrome (SJS) and toxic epidermal necrolysis (TEN)?

A

Starts w/non-specific sx - fever, cough, sore throat, sore eyes and itchy skin

Develops into purple/red rash that spreads across skin + blisters. Skin then starts to break away and sheds leaving raw tissue underneath

A

Medical emergency - admit to derm
Tx - steroids, immunoglobulins and immunosuppressant

A

A

IgE-mediated type 1 hypersensitivity reaction. When environmental allergens cause an allergic inflammatory response in the nasal mucosa.

* Hayfever
