Derm manifestations of IBD + appearance?

A

Pyoderma gangrenosum: begin as pustules, eroding and ulceration, central necrotic tissue DOESN’T ALWAYS CORRELATE WITH DISEASE ACTIVITY

Sweets: raised tender nodules (face, arm, trunk), neutrophils infiltration

Erythema nodosum: painful nodules on anterior legs/hands CORRELATE WITH BOWEL ACTIVITY

- itchy purpuric papules/vesicles/ bullae on extensor surfaces
- tx: dapsone, gluten free diet, sulfasalazine, colchicine

A

DO NOT DEBRIDE = gets worse

SLE derm changes?

A

Acute, subacute cutaneous LE (photosensitive, malar, maculopapular, bullous)
- Butterly rash: photosensitive, lightly raised, occ scaly erythema
- Spares nasolabial folds
- Generalised diffuse erythema

Subacute
- Annular/papulosquamous/both
- Photosensitive
- Infrequent scarring, hyper-hypopigmentation can occur

Chronic cutaneous LE (discoid lupus, panniculitis, lichen planus-like, hypertrophic verrucous, chillblains) - RO-52 associated
- Discoid: slightly raised, scaly hyperpigmented erythematous rims and depigmented, atrophic centers
- Scarring can occur
- Increased risk of systemic lupus, FBE abnormalities, positive ANA

Oral or nasal ulcers
Nonscarring Alopecia
- Diffuse
- Related to disease activity

Panniculitis
