Management of Acne

  • Non Pharm
  • Pharm for Comedonal vs Inflammatory
A

Non-Pharm

  • Mild non-soap cleanser OD
  • Avoid oil based cosmetics
  • Do not pick/squeeze the pimples
  • Avoid hot humid environments

Comedonal

  1. Benzoyl Peroxide 5% Topical
  2. Topical Retinoid - Tretinoin 0.025% topically, every second night for 2 weeks, wash off in am

Inflammatory

  1. Benzoyl Peroxide 5% Topical
  2. Antibiotic
    - Doxycycline 50-100mg OD or erythromycin 250-500mg BD for 6/52

Also
- Combined OCP - ethinyloestradial+cyproterone 35microg+2mg PO OD

  • Spironolactone 25-50mg PO OD

What are the side effects of Roaccutane (Isotretinoin)

?

A
  • Potent teratogen, thus requires contraception
  • May cause dry skin, eyes, oral mucosa, nose bleeds
  • Increased sensitivity to sunlight
  • Myalgias and joint stiffness
  • Cannot take tetracyclines due to incr risk of intracranial hypertension
