Derm Cases Flashcards
Pearly papule or nodule with dilated blood vessels
Basal cell carcinoma
Prognosis of malignant melanoma
depends on vertical thickness, measure by Breslow thickness. From granular layer of epidermis to deepest melanoma cell.
Solar alastosis
Change from normal to abnormal elastic fiber synthesis in dermis caused by sun damaged fibroblasts
Pagetoid spread
Spreading of individual melanocytes vertically throughout all layers of the epidermis
3 main epidermal cell types
squamous cells (keratinocytes), melanocytes, langerhans cells
Function of keratinocytes
protect body from fluid loss and mechanical injury, produce cytokines
Which epidermal cells are dendritic cells
langerhans cells
Gorlin syndrome
Nevoid basal cell carcinoma syndrome; have increased risk of developing BCC
Round nodular pearly tumor, telangiectasis, scaly crust
BCC
Significance of clefts between tumor nests and stroma?
Helpful in differentiating BCC from benign adnexal tumors like trichoepitheliomas
Name the benign adnexal tumors
Eccrine poroma, cylindroma, trichoepithelioma, syringoa, sebaceous adenoma
Ddx of superficial BCC
eczematous dermatitis, bowen’s dz, dermatomycoses (fungal infection), if pigmented- malignant melanoma
Do BCC metastasize?
Rarely
Actinic keratosis causes
sun damage, Ionizing radiation, hydrocarbons, and arsenicals
Actinic keratosis may progress to…
SCC in some cases. Some may regress spontaneously. Many remain stable.
Skin tumor that is well differentiated, but has atypical squamous epithelium proliferation, and surrounded by lymphocytic infiltrate
SCC vs. keratocanthoma- both present this way. Keratocanthoma- SCC with spontaneous regression
In situ SCC of skin and cause
Bowen’s disease, caused by HPV infection
Types of eczematous dermatitis
allergic contact dermatitis, drug-related eczematous dermatitis, seborrheic dermatitis, irritant dermatitis
