DMS 1/2 - 5 mph and Assistance is required if fault is in leading cab.
If the ‘Restriction’ is 10 mph below line speed but not required if line speed is below 35 mph, providing its not on the front or rear cab, what is the fault?
Fault:
Automatic Brake System.
Can remain in service provided only 1 coach is affected, and it is not the front or rear cab.
If the ‘Restriction’ is 40 mph when driving by yourself, but can remain at line speed if accompanied by a competent person. However, during darkness and poor visibility the 40 mph ‘Restriction’ must remain in place even when accompanied by a competent person, must what is the fault?
Fault:
Automatic Warning System (AWS)
Also, if the DSD is defective, the train cannot be moved without a competent person and the 40 mph ‘Restriction’ remains in place.
If the ‘Restriction’ is 5 mph, what is the fault?
Fault:
Driving Cab Controls - Brake Controller
If there is no Brake Continuity, then must not proceed at more than 5 mph.
If may be possible to either change ends and drive from the rear cab, or to be accompanied by a competent person and drive from the middle cab (Propelling).
If there is no speed ‘Restriction’, but you may need to be accompanied by a competent person in normal conditions as well as poor visibility or darkness what is the fault?
Fault:
Drivers Cab Windscreen - Damaged or Broken - To travel in Front cab.
EBS - Passengers to be transferred to leading cab - Driver to look back frequently to ensure train is still complete.
PassCom - Unable to reset - To travel in Front or Rear cab.
If there is no speed ‘Restriction’ and can continue without being accompanied by a competent person, what is the fault?
Fault:
Driving Cab Heating and AC - Driver to make judgement
Doors External - Proceed at caution - TIS and detrain at next station.
Driver Reminder Appliance - Driver to use own PPS.
Orange Body Side Lights - Check Interlock Button frequently.
Graffiti - Depending on severity - To be taken out of service.
Carriage Heating and Ventilation - Driver to make judgement.
Hustle Alarm - Driver to look back / Check Interlock Button frequently.
Lifeguards - Proceed at caution - Move to a suitable place to inspect - Authority from SLORM before any movement takes place.
OTDR - Driver to check OTDR is working in intermediate cab.
OTEE - Driver to check in another cab for equipment.
PA - Coach to be locked out of use if possible.
PIS - Driver to make announcements throughout journey.
Sanding Equipment - Driver to make judgement BUT not to enter service in Low Rail Adhesion conditions.
Speedometer - Driver to drive at speed which is within line speed.
Tail / Marker Light - Partial failure - Can remain in service
Tail Light - Complete failure - Arrange for lamp to be fitted
TIS - Driver to ensure all doors are closed as Interlock cannot be relied upon.
Train Internal Lighting - Coach to be locked out of use if possible.
Traction Motor - If loss more than 50% of power, train to be taken out of service.
Train Radio (GSM-R) - Not to be moved until instructed to do so.
WSP - Driver to make judgement on speed dependant weather and rail head conditions.
If the ‘Restriction’ is 40 mph, and not to move unless accompanied by a competent person, what is the fault?
Fault:
Driver Safety Device (DSD)
If unable to use Power Control DSD, then assistance from the front is required.
If AWS / TPWS is also isolated, then cannot move unless accompanied by a competent person.
If the ‘Restriction’ is 75 mph, what is the fault?
Fault:
Body Side Windows - Cracked or Broken
If only outer-pane 75 mph
If both panes - 75 mph and coach out of service
If the ‘Restriction’ is 75 mph but can also be reduced to 20 mph, what is the fault?
Fault:
Head Light - Complete failure
If Portable Headlamp fitted - 75 mph
If no Portable Headlamp fitted - 20 mph and frequent use of horn.
If the ‘Restriction’ is 20 mph maximum, but needs to be authorised before any movements take place, what is the fault?
Fault:
Locked Wheels / Wheel Flats / Shifted Tyres / Dragging Brakes
Slight Wheel Flats - No speed ‘Restriction’.
More Serious Wheel Flats - Call signaller immediately - 20 mph maximum BUT only on authority from DCM / Fleet.
Wheels Locked / Tyres Shifted / Wheel Flats greater than 60mm - NOT to be moved until inspected LORT who will advise on any speed ‘Restrictions’.
If the ‘Restriction’ is 10 mph but in some instances can be as little as 5 mph and up to 20 mph, what is the fault?
Fault:
Hot Axle Box
Confirmed Hot Axle Box - 10 mph on authority from DCM / Fleet / SLORM and NR - 5 mph over Points and Crossings.
If investigation is 20 minutes after activation or no initial evidence is found - Upon signallers instructions, a maximum of 20mph.

Fault:
TPWS - Not working when it should or working when it shouldn’t.

Fault:
Warning Horn - Full Failure
Permission needs to be given from NR - 20 mph.
Warning Horn - Partial Failure
No ‘Restriction’.
