Decon BDS5 Flashcards
What are the stages of the decontamination cycle?
- Acquisition - practices should have a documented policy for the purchase of any medical devices
- Cleaning - Automated cleaning is preferred to manual cleaning
- Disinfection - Reduce number of viable microorganisms
- Inspection - All instruments carefully examined for organic material and/or damage under magnification
- Packaging - Where products are to be packaged, the materials should be. Compliant with the relevant European Standards (BS EN 868)
- Sterilisation - The following should be taken into account; process used, type of cycle and relevant cycle parameters
- Transport
- Storage - Areas designated for storage should be clean, dry, well ventilated and secure. Stock rotation should be used FIFO.
- Use
- Transportation (dirty)
- Disposal
The location for decontamination processes may be divided into what three areas?
Dirty - cleaning of used devices
Clean - inspection, assembly and packaging
Clean - sterilisation and post sterilisation storage
What are the three main sources of contamination of a product prior to its sterilisation?
Facilities
Personnel
Raw materials/components
Who has the overall responsibility of the decontamination process?
Dental Practitioner/Owner (maybe referred to as the decontamination manager)
Practitioner must have undertaken periodic training to update their knowledge, skills and competence
What are policies?
When should policies be available for the decon process?
What are procedures?
How are procedures relevant to the decon process?
Policies describe the aim of a particular process
Should be available for all parts of the decon cycle including acquisition and disposal
Procedures describe how to achieve compliance with a policy
They should define, document and control all stages of the decon process
Who is the operator, user and manager in the decon process?
What are their roles?
Operator;
Defined as the person with the authority to operate a washer disinfector and steriliser
Can note readings, replace consumables and perform simple housekeeping duties
User;
Defined as the person designated by the practice owner/manager who is responsible for the decon equipment
Can certify equipment is fit for use
Holds all documentation
Ensures periodic testing and maintenance takes place
Can appoint operators
Can establish procedures
Manager;
Defined as the owner, employer, dental practitioner or practice manger who is ultimately acountable for operation of the dental premises
Ensures decon is carried out in compliance with the policies of the UK health departments and law
Ensures staff are trained
Ensures that purchased equipment complies to legal requirements and standards
Ensure that equipment is subject to periodic testing
Ensures procedures are documented and adhered to
Who is the authorising engineer in the decon process?
The AE(D) is defined as the person designated by HFS to advise on decontamination procedures, washer disinfector, sterilisers and associated sterilisation procedures
Principal responsibilities;
Give general and impartial advice on all matters concerned with decon
To audit reports
To advise staff on programmes of periodic tests and maintenance
Who is the competent person for decontamination - what are their responsibilities?
Who is the competent person for pressure systems - what are their responsibilities?
CP(D) is defined as the person designated to carry out maintenance, validation and periodic testing of washer disinfectors and sterilisers
Responsibilities are;
Carry out maintenance and repair tasks
Conduct validation and periodic tests
CP(PS) is defined in the pressure systems safety regulation 2000 as the engineer responsible for drawing up a written scheme of examination of a system e.g. porous load sterilisers
Define guidelines, standards and legislation
Guidelines;
Recommendations for the care of individuals by healthcare professionals based on the best available evidence
Standards;
A document established by consensus and approved by a recognised body…
Legislation;
Process of enacting statutory laws
What mark should decon devices bear?
What is the NP 143 contract? How often does it get updated?
CE mark
This is a NHS procurement (acquisition) department which is available to all general dental practices in Scotland who are contracted to the NHS
It is updated every 3 years
The advantage is that items from this contract are NHS approved and independently tested by the NHS
What are the key elements of the cleaning cycle?
Time
Temperature;
The temp of the initial wash is usually below 40 degrees to avoid coagulation and fixing of body tissues to instruments
Chemicals; Acidic detergents Neutral pH detergents Alkaline detergents pH 10 High alkaline detergents >pH10 Enzymatic detergents
Energy;
This is usually delivered in the form of physical energy in the water jets from the rotating spray arms.
What is the preferred back up cleaning method if an AWD is unavailable?
What is its documented procedure?
Ultrasonic cleaner
Correct loading procedure Correct choice of operating cycle time Operating temp Correct choice of detergent Draining of ultrasonic bath at 4 hourly intervals or sooner if grossly contaminated Record details of items processed
What does a typical washer disinfector cycle compromise of?
- Flush - remove gross contamination at <35 degrees
- Wash
- Rinse - remove residual detergent
- Disinfection
- Drying - purging the load and chamber with heated air
The management requirements for an LDU can be broadly divided into what categories?
- General management
- Risk management
- Health and safety
- Training
- Quality management system
What are the 8 principles of the ISO 9000 series of standards?
- Customer focus
- Leadership
- Involvement of people
- Process approach
- Systems approach to management
- Continual improvement
- Factual approach to decision making
- Mutually beneficial supplier relationships
