Injury and illness recording (29 CFR 1904)
Recording any injury and illness is required by law and the info is maintained on OSHA forms 300, 300A, & 301
Employers with 11 or more employees
Are required by OSHA to record keeping
Employers with less than 10 or less employees
Are expempt from record keeping unless selected by the Bureau of Labor Statistics to participate in the annual survey of Occupational injuries and illness

Recordable Occupational Injuries and illnesses
The employer must enter each recordable injury or illness on the osha logs within 7 calendar days of receiving info that it occurred
Any work related injuries and illnesses that result in one or more of the following must be recorded
Death Days away from work Restricted work Transfer to another job Medical treatment beyond first aid Loss of consciousness Diagnosis of a significant injury or illness
In-patient hospitalization, amputation, or eye loss must be reported….

Visits to a physician or other licensed health care solely for observation or counseling
