How are you going to Auscultate the heart
Verbalize where you’re putting the stethoscope
All - Aortic = 2nd intercostal space right sternal border
Physicians - Pulmonic = 2nd intercostal space left sternal border
Take - Tricuspid = 4th intercostal space left sternal border
Money - Mitral = 5th intercostal space midclavicular line
How are you going to inspect the carotids?
Tell the patient what you’re going to do.
Palpate the carotid pulse so you know where to listen - do not palpate bilaterally at the same time
use the bell of the diaphragm
ask the pt to take a deep breath and hold it, then listen for bruit on one side
tell patient it is OK to breathe
repeat on the other side.
What pulses are you going to palpate?
Brachial
Radial
Posterior Tibialis
Dorsalis Pedis
How are you going to check for edema?
Press with your fingertip for 5 seconds on the Dorsal of the foot, medial malleolus, anterior tibia
then release
Check for capillary refill
Apply pressure to fingernail or toenail for several seconds to cause blanching, release and check at least one digit bilaterally
verbalize that normal cap refill is less than 2 seconds
