MI acute with doses
Morphine 5mg IV Oxygen if hypoxic Nitrates sublingual tablet 500 micrograms Aspirin 300mg orally Ticagrelor 180 mg orally
Angina surgical
PCI (stent)
Coronary artery bypass (vein from leg)
Anginal Medical
GTN spray
Long term put on B blockers and CCB
AF
lots of choice
first line= beta blocker for rate and rhythm control
can also control rate with CCBs, digoxin
can also control rhythm with amiodarone
consider anticoagulation
Heart Failure
First line is ACEi and Beta Blocker
can add furosemide to help relieve symptoms
If stage 3/4 on the NYHA and the above aren’t working then consider:
- Aldosterone antagonists e.g. spironolactone (help mortality)
- ARB e.g. losartan
Consider digoxin if old and AF
MI secondary prevention
ACEi
Beta Blocker
Statin
Dual antiplatelet= aspirin and clopidogrel
Hypertension
ACEi if under 55 or diabetic
CCB if over 55 or black
Then combine
then add thiazide like diuretic
