What hormone is affected in cushings?
Cortisol

What investigations do you want for cushings?
- Overnight dexamethasone suppression test or 24hr urinary cortisol
- 48hr dexamethasone suppression test. If abnormal:
- Localise legion - plasma ACTH, High dose dexamethasone suppression test (some response suggests cushings disease)
- Imaging - MRI pituitary, CT chest and pelvis

Proximal myopathy - shrug shoulders against resistance

Causes of Cushing’s syndrome
High ACTH:
- cushings disease - pituitary adenomyosis
- ectopic ATCH - SCLC
Low ACTH (excess cortisol)
- adrenal adenoma/carcinoma
- steroid use
Concluding a Cushing’s exam
Is the pt on steroids? No:
Look for cause: see Ix
Assess visual fields
Other tests: urine and BM - DM, bone scan, U&E - hypoK
