What is CTG?
Method of recording foetal heart rate and uterine contractions during pregnancy
What do the top and bottom lines represent on CTG?
Top - foetal heart rate
Bottom - uterine contractions
What is the normal foetal heart rate?
110-160bpm
What are the four parameters for describing contractions?
- Frequency (no. contractions/10 mins)
- Duration (seconds/minutes)
- Strength (mild/moderate/strong)
- Consistency (regular/irregular)
How is CTG interpreted? (DR. C BRAVADO)

How long is a standard trace?
20 minutes (running at 1cm/min)
What is variability on CTG?
Oscillations on the trace - normal variation is 5-15bpm (1-3 small boxes)
What are accelerations on CTG? What acceleration rate is normal?
Rise from baseline >15bpm lasting 15 seconds
2 accelerations/20 mins is good
What are decelerations on CTG?
Decrease from baseline >15bpm lasting 15 seconds
What are the three types of decelerations and their causes?
- Early - head compression
- Variable - cord compression
(typical or atypical) - Late - foetal hypoxia
What does a sinusoidal CTG pattern indicate and how is it managed?
Foetal hypoxia - emergency C-section
How is a non-reassuring CTG managed?
Maternal position change
Oxygen and IV fluids
Continuous CTG and vitals monitoring
Assess cervix
Stop oxytocin
Tocolysis
Plan for delivery
