2
Q

A cosa è dovuto il distacco sieroso del NE?

A

Leakage da coriocapillari attraverso difetti focali (o – aree di iperpermeabilità) dell’EPR

Q

Esordio

A
Metamorfopsia unilaterale (+ micro, mild discrom e < sens contrasto)
                               \+

13
Q

Ridotta VA è correggibile?

A

Spesso sì, con l’ausilio di debole lente convessa, a causa dell’ipermetropia acquisita da elevazione del NE

16
Q

Fundus OCT

A

Subretinal fluid + almeno 1 abnormal depigmented RPE focus fino a DEP, talora piccoli e multipli

(+ small patches of EPR atrophy/hyperplasia elsewhere in post pole [site of previous lesions?])

17
Q

Diagnosi

A

Amsler +

OCT: optically empty NE elevation +/- DEP/deficit EPR

18
Q

FAG

A
SMOKESTACK (early iper spot vertical column + diffusion late)
INK BLOT (early iper spot that gradually enlarges)
19
Q

ICGA

A

Early: dilated/compromised chor vv at post pole
Mid: areas of pier per chor iperpermeabilità

20
Q

Decorso

A

1) Risoluzione spontanea (max parte pz, w/in 3-6 mm, con progressivo&raquo_space;> VA ma recidive in 1/2 casi)
2) Cronicizzazione (–, > 12 mm, older pz)

21
Q

Treatment

A

1) Observation (++)
2) Sospensione terapia steroidea (++ forme croniche/ricorrenti, in cui utile anche PDT)
3) Lifestyle change (< stress!) in selected pz
4) Laser fotocoag all’EPR leak site (speeds resolution, ma no impact on final VA/recurrence) –> in caso, aspettare almeno 4 mm; controindicata se leak in FAZ

EXP: IVL anti-VEGF
