Safety: Check reading, help, scan surgery/pt/monitor, stop surgery
Temporise: O2, Fluid, pressors, others e.g. position change
Diagnosis: most likely cause, serious differentials to rule out, systematic approach
Postop: Documentation/Debrief/open disclosure/incident reporting/contact medicolegal advisor
