2/3:
- GCS < 15
- Systolic BP < 100
- RR > or =22
King’s College Criteria
liver transplant in acetaminophen toxicity
pH < 7.35 s/p fluids for 12 hrs
OR
- INR > 6.5 PT > 100
- encephalopathy grade III or IV
- Cr > 3.4
Centor
1 - no cough
2 - fever
3 - pharyngeal exudate
4 - tender LNs
2 - rapid strep and cx
3 + - empiric tx
CHADS2
C - CHF H - HTN A - age > 75 D - DM S - stroke / TIA
2 + - A/C for a fib
HAS -BLED
H - HTN
A - abnormal liver or renal function
S - stroke
B - bleeding hx or predisposition
L - labile INR
E - elderly (> 65)
D - drugs / alcohol use
If score > CHADS 2 then no A/C for a fib
PERC
- age > 50
- HR > 100
- SaO2 < 95%
- Unilat leg swelling
- Hemoptysis
- Recent surgery or trauma in last 4 wks
- Prior DVT or PE
- Hormone use - OCPs, estrogen
Well’s Criteria
- Sign/ sx DVT (3 pts)
- PE is #1 diagnosis (3 pts)
- HR > 100 (1.5 pts)
- 3 days immobilization or surgery in last 4 wks (1.5 pts)
- Prior DVT or PE (1.5 pts)
- Hemoptysis (1 pt)
- Cx treated within 6 mo or palliative (1 pt)
< 2 D-dimer or PERC
> 2 CT -angio
HEART Score
- Hx - suspicious?
- EKG - normal / non-specific / ST elevations
- Age < 45 / 45-64 / > 65
- RF (DM, tobacco use, HTN, HLD, family hx)
- Initial troponin
If 3 or less … d/c
If 4-6 admit
If > 6 intervene
NIH Stroke Scale Components
- level of consciousness
- orientation to mo and year
- blink eyes and squeeze hands
- horizontal gaze (EOM)
- visual fields
- facial palsy
- drift in ea limb (scored sep)
- limb ataxia
- sensation
- aphasia
- dysarthria
- inattention
Ottowa Knee Rules
age > 55
isolated patellar tenderness
tenderness at head of fibula
cannot flex knee 90 degrees
cannot bear wt
Ottowa Ankle Rules
Tenderness at base of 5th metatarsal
Tenderness at posterior medial or lateral malleoli
unable to bear weight
Canadian Head CT Rules
GCS < 15 2 hrs after injury
Signs skull fracture (raccoon eyes, csf leak)
2+ episodes vomiting
age > 65
Head lac / skull hematoma
30+ min amnesia
on A/C
Dangerous mechanism
Neuro deficits or seizure

For fluid resuscitation after burns …
4mL x kg x %TBSA
Give 1/2 in first 8 hrs since injury (NOT presentation)
Give 1/2 in next 16 hrs

Predicts stroke risk in 2 days after TIA
A - age > 60 ... 1 pt B - BP (systolic> 140 or diastolic > 90)... 1 pt C - clinical features - unilateral weakness ... 2 pt - speech impairment without weakness ... 1 pt - other ... 0 pt D - duration - > 60 min ... 2 pt -10 - 59 min ... 1 pt - < 10 min ... 0 pt D - DM ... 1 pt
6-7 pts = high risk
4-5 pts = moderate risk
0-3 pts = low risk
