Cranial nerve 1
Olfactory- smell
Test: Smell with eyes closed
Cranial Nerve 2
Optic- vision and visual fields
Test: Snellen Chart
Cranial Nerve 3
Oculomotor- Extraocular movement; movement of sphincter or pupil; movement of cillary muscles of lens
Test: PERRLA, 6 ocular movements
Cranial Nerve 4
Trochlear- moves eyeball downward
Test: PERRLA,
Cranial Nerve 5
Trigemenial- Sensation of cornea, skin on face, and nasal mucosa
Test: sensation of face cornea, clench teeth and open mouth against resistance

Abducens- EOM; moves eyeball laterally
Test: Perr
Cranial Nerve 7
Facial- Facial expression; taste
Test: smile, raise, eyebrows, frown, puff out checks and taste
Cranial Nerve 8
Cochlear-Hearing; air conduction and bone conduction
Test: Watch tick test, Weber’s and Rinne tests
Cranial Nerve 9
Glossopharyngeal- swallowing ability, tongue movement
Test: say ahh and watch if palate elevates, and taste
Cranial Nerve 10
Vagus- sensation of pharynx and larynx; swallowing
test: swallowing and speech for hoarseness
Cranial Nerve 11
Accessory- Head movement; shrugging shoulders
Test: Head movement side to side; shoulder shrugging
Cranial Nerve 12
Hypoglossal- protrusion of tongue; move tongue side to side and up and down
Test: stick tongue out and move it up down, side to side
