Cranial Nerve I Exam
Olfactory
-Occlude one nostril and close eyes. Identify presented smell. Repeat other nostril.
Cranial Nerve II Exam
Optic
- Cover one eye and perform Snellen eye chart. Repeat other eye.
- Visual fields test (peripheral vision wiggly fingers)
- Shine light in pupils (PERRLA)
Cranial Nerve III, IV, VI Exam
Oculomotor, Trochlear, Abducens
-Six cardinal fields of gaze (follow finger in star pattern)
Cranial Nerve V Exam
Trigeminal
- Close eyes. Light, bilateral touch to forehead, cheeks, jaw.
- Clench teeth and move jaw side-to-side
Cranial Nerve VII Exam
Facial
-Smile, frown, raise eyebrows, puff cheeks
Cranial Nerve VIII Exam
Acoustic
-Whisper test each ear or rub fingers together
Cranial Nerve IX, X, XII Exam
Glossopharyngeal, Vagus, Hypoglossal
- Stick out tounge and move side-side
- Say “Ahhh” to inspect midline uvula
- Swallow
Cranial Nerve XI Exam
Spinal Accessory
- Place hand on patient’s cheek and have turn head towards examiner hand
- Shrug shoulders against examiner hands

-Moves eyelid, eyeball, pupil, lens

-Moves eyeball

-Moves facial muscles, chewing, facial sensation

Abducens
-Moves eyeball
Cranial Nerve VII
Facial
-Taste, tears, saliva, facial expressions
