Optic - visual acuity, visual fields
Test acuity, test visual fields by confrontation
CN III
Oculomotor - pupillary reactions (IO, SR, MR, IR)
Palpebral fissure alignment, pupil size, regularity, light reaction and accommodation, Cardinal positions of gaze.
CN IV
Trochlear - extraocular movements (SO)
Palpebral fissure alignment, pupil size, regularity, light reaction and accommodation, Cardinal positions of gaze.
CN V
Trigeminal - corneal reflexes, facial sensation and jaw movements
Motor function: palpate temporal and masseter muscles as person clenches jaw.
Sensory: touch lightly on all three dermatomes with pt’s eyes closed
CN VI
Abducens - extraocular movements (LR)
Palpebral fissure alignment, pupil size, regularity, light reaction and accommodation, Cardinal positions of gaze.
CN VII
Facial - Facial movements
Note facial mobility and facial symmetry for smile, frown, show teeth, puff cheeks and resist eyes from opening against force, raise eyebrows
CN VIII
Vestibulocochlear - hearing
Assess hearing acuity, perform Weber and Rinne if necessary
CN IX
Glossopharyngeal - swallowing, rise of palate, gag reflex
Assess ability to swallow, hoarseness, nasal quality
Say ah, watch palate and uvula rise and fall
Touch back of throat for gag reflex
CN X
Vagus - swallowing, rise of palate, gag reflex
Assess ability to swallow, hoarseness, nasal quality
Say ah, watch palate and uvula rise and fall
Touch back of throat for gag reflex
CN XI
Accessory - movement of trapezius muscles
Shrug against my hands, turn head to each side against my hands
CN XII
Hypoglossal - voice and speech, tongue movement
Listen to speech, ask pt to stick out tongue note asymmetry or deviation from the midline
CN I
Olfactory - Smell
Present pt with familiar odors
