Snellen Chart (Eye Chart)
Oculomotor Nerve Assessment
Use penlight to assess Pupillary Constriction and make note of any visible eyelid drooping

Trochlear Nerve
Trochlear Nerve Assessment
Movement of eye down towards nose.
To assess, instruct patient to follow your finger while you move it down to- ward his nose.
Olfactory Nerve Assessment
Patient uses sense of Smell
Spinal Accessory
Assessing ability to shrug shoulders & turn head against pressure
Facial Nerve Assessment
~Facial Symmetry~
Hypoglossal Nerve Assessment
~Involves Tongue~ (Often combined with CN IX & CN X assessments)
Have pt. stick out tongue, should be midline
Look for difficulty eating, swallowing, or speaking
Abducens Nerve Assessment
Movement of Eye toward each ear (Outward)
CN I: Olfactory Nerve
CN II: Optic Nerve
CN VIII: Aucoustic Nerve (aka:Vestibulocochlear Nerves)
Sensory Nerves
Trigeminal Nerve Assessment
Involves most of the face • Forehead • Cheek • Jaw Facial Sensation (Sharp/Dull recognition) & Jaw Strength Assessment
CN III: Occulomotor Nerve CN IV: Trochlear Nerve CN VI: Abducens Nerve CN XI: Spinal Accessory Nerve CN XII: Hypoglossal Nerve
Motor Nerves
Acoustic Nerve Assessment
~Assessing the ability to hear & Balance Disorders r/t the inner ear are often assessed at this point.
- Whisper Test
- Romberg Test
Trigeminal Nerve Assessment
Nurse will ask the patient to close their eyes & identify sensations of the face when touched. (Identify Sharp/dull sensations.
- forehead
- cheeks
- jaw strength
Glossopharyngeal & Vagus Nerve Assessment
~Assess gag reflex by asking pt. say “ahh”
~Touching the back of throat with depressor
~Ability to taste Salt, Sweet, & Sour.
- Whisper Test- whisper into pt. ear and repeat what was said.

CN VII: Facial Nerve
CN IX: Glossopharyngeal Nerve
CN X: Vagus Nerve
