Olfactory (s), test smell
Cranial Nerve 2 (II)
Optic (s), test peripherals and near/ far sight
Cranial Nerve 3 (III)
Oculomotor (m), up and down eye movements and pupils
Cranial Nerve 4 (IV)
Trochlear (m), eye movement down and away (X)
Cranial Nerve 5 (V)
Trigeminal (b), Branches: Ophthalmic, Maxillary and Mandibular. Test sensory for branches and clench jaw for motor.
Cranial Nerve 6 (VI)
Abducens (m), side to side eye movement
Cranial Nerve 7 (VII)
Facial (b), test facial expressions (smile, frown, puff cheeks, raise eyebrows) and test sensory posterior cheek
Cranial Nerve 8 (VIIII)
Vestibulocochlear (s), test balance and hearing (rub fingers together and move away from ears)
Cranial Nerve 9 (IX)
Glossopharyngeal (b), quality of voice and ask patient to swallow
Cranial Nerve 10 (X)
Vagus (b), ask patient to swallow.
Cranial Nerve 11 (XI)
Accessory (m), trapezius and sternocleidomastoid (ask patient to shrug shoulders)
Cranial Nerve 12 (XII)
Hypoglossal (m), ask patient to stick out tongue (looking for any deviations)
