Cranial Nerve I
Olfactory— Smell
Access by asking patient to close eyes and identify a common smell. Ie coffee peppermint
Cranial Nerve II
Optic— sight
Assess by having patient identify what they see on a chart
Cranial Nerve III
Oculomotor- eye movements
Assess by standing 1 foot away and asking patient to follow direction of the light with only their eyes
Cranial Nerve IV
Trochlear- moves eye
Assess by
Cranial Nerve V
Trigeminal- Face Sensation
Assess by asking patient to close eyes and gently wisp a cotton ball lightly touching their face, forehead, chin. Instruct patient to say now every time they feel it.(Sensory)
Assess motor function by having patient clench teeth tightly while palpating temporal and Masseter muscles. Ask them to open and close mouth several times.
Cranial Nerve VI
Abducens- abducts eyes
Assess by asking patient to look left and right eye should look to their side not inwards
Cranial Nerve VII
Facial- assess by asking patient to smile, show teeth, look for symmetry.
Sensory function assess taste
Cranial Nerve VIII
Vestibulocochlear- Auditory
Assess by performing the whisper test as well as using the Romberg test to test balance.
Cranial Nerve IX
Glossopharyngeal- swallowing salvia taste
Assess by asking patient to open mouth and say ahh check uvula and tongue
Cranial Nerve V
Vagus- control of PNS
Assess by checking gag reflex
Cranial Nerve XI
Accessory- moving head and shoulders
Assess by placing hand on patients forehead and pushing backward as patient pushes forward
Cranial Nerve XII
Hypoglossal- tongue muscles
Assess by asking patient to stick out their tongue
