Cranial nerve 2 test
A
Pupillary light reflex (optic nerve)

How is the pupillary light reflex test performed?
A
- observe resting pupil size symmetry
- shine bright light in one eye inferiorly or laterally and observe constriction
- observe indirect constriction of other eye

Cranial nerve 3, 4 and 6 test
A
Visual pathway and eye movement (occulomotor, trochlear and abducens)

How to perform CN3,4 and 6 test
A
- patient covers 1 eye and you put fingers up in all 4 quadrants one at a time
- move hand more than 1 foot from midline in each quadrant

A
Gag reflex (glossopharyngeal)

How to perform CN9 test
A
- have patient open mouth and use tongue depressor
- gently touch uvula with cotton swab

A
Palatal function (vagus nerve)

How to perform CN10 test (vagus nerve)
A
- have patient say “ah” and observe bilateral elevation of palate

Test for CN11
A
Sternocleidomastoid and Trapezius strength (accessory nerve)

How to perform CN11 test
A
- Ask patient to turn head to one side, put hand on chin and attempt to overcome them turning head to other side
- Ask patient to shrug shoulders, push them down and tell patient to resist

Cranial nerve 12 test
A
tongue movement (hypoglossal nerve)

How to perform CN12 test
A
- ask patient to stick tongue out and look for symmetry
- ask to wiggle one side and other
- look for fasiculations

CN5 tests
A
- facial sensation (V1,2,3)
- Jaw strength
- sensation and motor of trigeminal

How to perform CN5 facial sensation test
A
- lightly touch both sides of forehead simultaneously
- do same with cheeks and chin
- should be equal bilaterally in all 3 regions
