Duty to investigate- following a death, the coroner must conduct an investigation if theere is any reason to suspect:
- violent or unnatural death
- cause of death is unknown
- deceased died while in custody or state detention
“notifications of death regulations”- a registered medical practitioner must notify the coroner if a person deaths:
a) the patients death was due to:
poisoning
exposure to toxic substances
controlled or medicinal product
violence and self neglect/ neglect
self-harm etc.
b) suspect was unnatural
c) cause if death is unknown
d) the person died in custody
e) there is ni attending medical practitioner to sign a MCCD
f) attending isnt available in time to sign the MCCD
g) identity of the deceased cannot be identified
what is a MCCD
medical certificate of cause of death
who can issue a MCCD
medical practitioner if:
- patient died of natural casues
- the doctor has seen the patient within 14 days or has seen the body after death

alternatively take solemn affirmation to same effect
