COPD Pharmacology Flashcards
Which drugs are used in acute exacerbations of COPD?
-bronchodilators
-corticosteroids
-antibiotics (short-term)
Why are dose increases (in magnitdue) of bronchodilators NOT recommended in treatment?
-flat dose response curve (dose increase does not equal increased response)
-dose increase is related to toxicity
What is the onset of action of SABAs?
~5-8 minutes
What is the duration of action of SABAs?
2-6 hours
What is the DOA of LABAs?
> 12 hours
What are some ADR of beta agonists (SABAs/LABAs)?
-tachycardia
-tremor
-hypokalemia
-hyperglycemia
-tachyphylaxis
What are some ADR of muscarinic antagonists
-dry mouth
-metallic taste (tiotropium)
-cough
-nausea
-blurred vision
What is the mechanism of action of Roflumilast?
PDE4 inhibitor
What is the mechanism of action of Ensifentrine?
PDE3 and PDE4 inhibitor
What are some ADR of Roflumilast?
-N/D
-weight loss
-sleep disturbances
-HA
-may worsen depression and suicidal ideation
What DDIs does Roflumilast?
-3A4 inhibitors
-3A3 and 1A2 inducers
What drug do you NOT use Roflumilast with?
Theophylline
In what patient population do we want to be careful with Roflumilast use?
frail patients and those with a history of depression
What are some ADR of Ensifentrine?
-hypertension
-diarrhea
-back pain
What is the MOA of Dupilumab?
-human MoAb
-blocks shared receptor for IL-4 and IL-13 which reduces inflammatory response
What are some ADR of Dupilumab?
-injection site reaction
-infections (viral/URI)
-development of neutralizing antibodies
What should you avoid with Dupilumab?
live vaccines
