Main management of COPD
Smoking cessation
Influenza and pneumococcal vaccinations
Mode of action of muscarining M-receptor antagonists
Block parasympathetic bronchoconstriction
Given by inhalation to limit antimuscarinic side effects
BEta blockers and COPD?
COpD should not be a reason to avoid beta blockers where there is a compelling need
If beta blockers are prescribed, need to monitor respiratory function
First line treatment
Short acting antimuscarininc or Short acting beta agonist
Second line treatment (without asthmatic features)
Replace short acting antimuscarinic with a long acting antimuscarininc and LABA
Second line treatment with asthmatic features
LABA + Inhaled corticosteroid
Third line treatment? When used and what to use
When there is hospitalisation or 2 episodes (oral steroids or antibacterials)
Add inhaled corticosteroids
Third line treatment with asthmatic features
Add LAMA
How to treat exacerbations
Antimicrobial therapy
