Valid - 6 months
Quantity Limits - 30 day (90 with valid medical reason, can’t be ICD code)
Refills - none
Physicians - Schedule III-IV limits
Valid - 6 months
Quantity - 90 days
Refills - up to 5 times within 6 months
Physicians - Schedule V limits
Valid - 1 year
Quantity - 90 days
Refills - PRN
Physicians - Non-controls limits
Valid - 1 year
Quantity/Supply - as prescribed

APRNs - Schedule II limits
hydrocodone products only - limited to 5 day or 120 hour supply *includes single ingredient products
