Standard Precaution (8 Steps)
PROCEDURE STEPS
1. Verifies type and purpose of isolation
2. Applies appropriate PPE
3. Enters room, identifies patient, introduces self and department, and performs necessary task
4. Discards contaminated supplies appropriately
5. Removes PPE and follows universal precautions
6. Washes/gel hands
7. Re-gloves to clean or transport equipment to contaminated area
8. Discards gloves and re-washes/gel hands

- Surgical or N95 Mask
- Booty’s (Shoe Covers)

Incentive Spirometry (14 Steps)
PROCEDURE STEPS
1. Verifies physician’s order / facility protocols
2. Scans chart for appropriateness of therapy
3. Utilizes and maintains standard precautions as required
4. Gathers and assembles equipment
5. Identifies patient, introduces self and department
6. Assesses patient before therapy and places in proper position
7. Explains / instructs procedure to the patient and confirms patient’s understanding
8. Monitors patient throughout therapy for side effects
9. Instructs the patient to cough
10. Assesses the patient post procedure
11. Leaves the device within the patients reach and encourages to do on own
12. Thanks patient and ensure safe and needs are met
13. Disposes of PPE; washes / gel hands
14. Documents

Incentive Spirometry
Procedure Explanation & Freq
- Patient Sit-up
- Take in breath and exhale completely
- Put mouth around mouthpiece and ensure tight seal once breath is all out
- Take in a slow deep breath (Keep indicator between two lines) until lungs are filled
- Hold breath for 5-10 seconds
- Exhale completely all air in lungs
*Freq: 1. 10 Breaths Q H while awake (10 breaths “every hour” while awake)
2. QID (4 times a day)
Directed Cough Techniques: This check-off is to be combined with the therapy/treatment check-off
(12 Steps)
PROCEDURE STEPS - Patient Preparation
1. Explains purpose of procedure
2. Places patient in appropriate position
3. instructs patient on incisional support (if applicable)
4. Encourages a deep inspiration and hold
5. Instructs the patient in serial coughing techniques
6. Instructs patient on forced expiratory technique (FET), or huffing
7. State / Discuss purpose and technique of Quad Cough. If applicable, perform
8. Reassesses patient and reinstruct any of above as needed
9. Collects and examines sputum (if applicable)
10. Thanks patient, ensures patient comfort, safety, and needs
11. Disposes of PPE; washes/gel hands
12. Document with therapy / treatment
