List the limitations and possible side effects of the IUS?
frequent uterine bleeding and spotting painful insertion infection (but smaller risk than IUD) ectopic pregnancy dysfunctional ovarian cysts expulsion
How does the copper IUD work?
T shaped device made of copper
decreases sperm motility and survival
What are the benefits of IUD?
99% effective
long term - no worries regarding compliance
reversible
can be used as emergency contraception as effective immediately
When does the IUS start working as a contraceptive?
7 days after insertion
What are the limitations of IUD?
painful insertion heavier, longer, more painful periods infection - PID uterine perforation ectopic pregnancy expulsion
Name an example of the progestogen only subdermal implant
Nexplanon
How does the progestogen only implant work?
inhibit ovulation * by slowly releasing progestogen hormone etonogestrel
thicken mucus
thinning of the endometrium
prevent implantation
How/when is the progestogen only implant inserted?
inserted in the proximal non dominant arm, overlying the tricep
insert in day 1-5 of womens cycle - if not, need contraception
can be inserted immediately after termination of pregnancy
What are the benefits of the progestogen only implant?
highly effective
convenient
no oestrogen so can be used in patients with a history of VTE
What are the possible side effects with the progestogen only implant?
irregular/heavy bleeding *
‘progestogen effects” - headache, nausea breast pain
changes in weight
change in mood and libido
List the contraindications for the progestogen only implant?
ischaemic heart disease
breast cancer
liver cirrhosis or liver cancer
enzyme inducing drugs e.g. anti-epileptic, rifampicin
