Which situations should the high dose OCP be reserved for?
A
Breakthrough bleeding on lower dose COCPs
Control on menorrhagia
Concomitant use of enzyme inducing drugs eg epilepsy
Low dose pill failure

What is the usual age to stop the OCP?
A
50-51. Wait several weeks after stopping then measure serum FSH and oestrodiol levels. If oestrodiol levels low and FSH high the woman can be regarded as menopausal and can start HRT if desired

A
First 2 weeks postpartum History of thromboembolic disease including thrombophilia Cerebrovascular disease Focal migraine Coronary artery disease Oestrogen dependent tumours eg breast Active liver disease Polycythemia

A
Heavy smoking age >35 and smoker (or other risks of CAD) Undiagnosed abnormal vaginal bleeding Breast feeding 4 weeks before surgery 2 weeks after surgery Gallbladder or liver disease Hypertension Diabetes mellitus Long term immobilisation Complicated valvular heart disease Hyperlipidaemia Chloasma Severe depression

Known or suspected pregnancy Active PID Undiagnosed abnormal genital tract bleeding Previous ectopic pregnancy Severe uterine cavity distortion

A
Menorrhagia Dysmenorrhea Lesser uterine cavity distortion Very large or very small uterus Anaemia Defective immune system Impaired clotting mechanism Valvular heart disease Acutely anteverted or retroverted uterus Increased risk of PID (multiple sex partners)
