Which CGM will OHP cover? What’s required for the PA (prior authorization) to be approved?

A

Freestyle Libre 2

To qualify for CGM coverage through OHP, a patient must:

  1. Have diabetes mellitus
  2. Take insulin or have a history of low blood sugar problems
  3. Have a prescription for testing supplies and instructions on how often to test your blood glucose

*Use this info to educate customers. CGM’s can help patients retain quality years later in life, and when combined with a pump, they are a GAME CHANGER. Someday this pseudo-organ combination will be the gold standard.

What qualifies an OHP patient for an insulin pump?

A

To qualify for an insulin pump under Oregon Medicaid, a patient must meet the following criteria:

  1. Complete a diabetes education program
  2. Have been on a multiple daily injection regimen for at least six months
  3. Document glucose self-testing at least four times per day
  4. Meet one or more of the following criteria:

-Glycosylated hemoglobin level (HbA1C) greater than 7%
-History of recurring hypoglycemia
-Wide fluctuations in blood glucose before mealtime

A

Two Components requiring separate prescription:

  1. Freestyle Libre 2 Reader (optional)

—the freestyle libre 2 app for androids and IPhones replaces the reader

—only Medicare patients are required to buy and use the reader for insurance coverage

—If patient isn’t on Medicare, I recommend calling to verify and placing the reader on hold

  1. Freestyle Libre 2 Sensors

—One box = one sensor for a 14 day supply

What are the differences between the Freestyle Libre 2 and Freestyle Libre 3?

A

The 2.1 app update for the Freestyle Libre 2 allows the sensors to read and transmit blood glucose levels to the users phone every minute like the Freestyle Libre 3.

Other differences between the products are not clinically significant.

What substances could interfere with the results generated by the FreeStyle Libre system?

A

Taking Vitamin C (ascorbic acid) while wearing the Sensor may falsely raise the Sensor glucose readings—dose dependent

Taking aspirin (salicylic acid) may slightly lower the Sensor glucose readings—dose dependent

Dehydration will cause inaccurate readings
