Contact Len Fitting philosophies Flashcards
Soft CL fitting is based on?
THREE-POINT TOUCH:
Corneal Apex
Limbus
Peri-limbal Region
Remedy if Lens has Tight/Steep fit:
Flatten BOZR
Reduce TD (OAD)
Remedy if lens has Loose / FLAT FIT:
Steepen - BOZR
Increase - TD
Remedy if Lens is DECENTERED:
Increase TD
Comfort level if Tight / Steep Fit:
Comfortable at First
Comfort level if LOOSE / FLAT FIT
Uncomfortable
Centration if LOOSE / FLAT FIT:
Decentered / off-center
produces corneal exposure
Vision when blinking if TIGHT / STEEP FIT:
BOV - after blink
Vision when blinking if LOOSE/FLAT FIT:
Clear Vision - after blink
Push-up test if TIGHT / STEEP FIT:
Resistance to Movement - Sluggish recovery
Push-up test if LOOSE/FLAT FIT:
EXCESSIVE movement - erratic recovery
Retinoscopic Reflex if TIGHT/STEEP FIT:
Crisp & Clear
Retinoscopic reflex if LOOSE/FLAT FIT:
BLURRED REFLEX
Primary Gaze movement amount if Optimum fit:
0.2 - 0.4 mm
IDEAL MOVEMENT
Primary Gaze movement if TIGHT/STEEP FIT:
less than 0.2 mm
Primary Gaze movement if LOOSE/FLAT FIT:
greater than 0.4 mm
Vision if contact lens has Optimum fit:
Crisp. Clear. Stable
Vision if the contact lens Has tight / steep fit:
Clear.
but not stable
Vision when contact lens has a Loose / flat fit:
only clear after Blinking,
Variable vision after blinking
Corneal Coverage if CL has Optimum fit:
Full coverage in primary gaze at ALL times
with @least 1-2mm overlap
Corneal coverage if Tight / Steep fit:
Full coverage but not at all times
Corneal Coverage if Loose / flat fit:
NOT full
Corneal exposure @ extremities of gaze
Edge alignment if lens has Optimum fit:
Regular & Uniform alignment to the conjunctiva all throughout
Edge alignment if tight / steep fit:
Regular & uniform all throughout
BUT
has Conjunctival indentation or Peri-limbal imprint
(causes stagnation of tears & reduced Oxygen supply)
Edge alignment if LOOSE / FLAT FIT:
NOT regular
edge stand-off or Fluting
Lens Lag if Optimum fit:
no lens lag
Len lag if tight / steep fit:
no lens lag
Lens lag if loose / flat fit:
Excessive
especially at extreme gaze
