What is Comprehensive Orthodontic Treatment?
- Fixed orthodontic appliances
- May involve extractions with or without orthognathic surgery
- May involve other disciplines
When do we procede from Phase I to Phase II?
Usually after permanent second molars erupt
What is the duration of Phase I treatment?
1 year + Retention
When do you start growth modification?
Depends on malocclusion type
Patient in late mixed dentition stage
When do you start orthognathic surgery?
Until growth cessation
Except progressive deformity from growth restriction (TMJ ankyloses after trauma or infection)
Severe psychosocial issues
Types of Brackets
- Conventional Edgewise metal bracket
- Self-ligating bracket
- Ceramic brackets
Components of Edgewise appliances
- Brackets with/without bands
- Arch wires
- Elastomeric modules or wire ligatures
- Brackets are bonded to tooth surface with composite resin adhesives
Advantages of Aligners
- Esthetics
- Ease of oral hygiene
- Minimal dietary restrictions
- Control of tooth movement rate
Disadvantages of Aligners
- Compliance-dependent
- Certain tooth movements can be challenging
Examples of Treatment Auxillaries
- Growth modification
- Anchorage
- Elastics
What should you do before orthodontic treatment?
- Develop multidisciplinary treatment plan and sequence
- Disease control
What should you do during orthodontic treatment?
- Good OH control, regular dental check-ups and prophylaxis
- Debond after confirmation form other providers
- Immediate retention
What should you do after orthodontic treatment?
Definitive restorative and periodontal treatment
When is Growth Modification usually performed?
Before edgewise appliances
May need time interval growth modification and fixed appliance e.g. post-expansion
Re-evaluation of treatment plan after growth modification
