Define the CGA
‘A multidimensional interdisciplinary diagnostic process focused on determining a frail elderly person’s medical, psychological and functional capability in order to develop a coordinated and integrated plan for treatment and long-term follow-up’
Is a CGA done all at the same time?
No not necessarily - an MDT meeting can help to get all this info together
What are the components of the CGA?
PHYSICAL, MENTAL, FUNCTIONAL, SOCIAL Physical symptoms Mental Health Functional abilities Living environment Social circumstances
What factors might come under physical symptoms?
Patient's presenting complaint Incontinence Mobility Pain Sensory impairment Falls assessment - walking aids Nutritional and skin assessment
What factors might come under mental health?
Mood
cognitive impairment - new or old
What factors might come under functional ability and living environment?
Are they managing at home
Home modifications
What factors might come under social aspects?
Carers
Family - what are they able to offer?
Which members of the MDT are involved in the CGA
everyone eg nurses doctors physio OT care home staff social worker
Explain what case management is and what it might involve?
case management is about coordinating the care plan
led by a case manager eg a community matron
the case manager is aware of the pt’s goals and when the pt needs to be reviewed
What are the challenges in delivering CGA in the community?
- not all staff under one location
- bridge between health and social care (NHS and private companies)
- high staff turnover
- lack of centralised IT system
What are the positive outcomes of CGA?
- reduced hospital admission
- reduced mortality at 12 months
- positive pt and carer experience
- more appropriate prescribing
