A

Used to identify malnutrition in adults

Score:
BMI
% unplanned WL
Active illness

0 = low risk
1 = medium risk - observe intake for 3 days (if adequate monitor regularly, if inadequate see below)
≥2 = high risk - treat: 
Refer to dietician
Set goals it improve intake
Monitor regularly
