Complication of skin RT
What are the complications of skin radiotherapy?
Acute-
• Erythema - aqueous cream or soft paraffin
• Dry desquamation- 1% hydrocortisone cream
• Moist desquamation - paraffin gauze or hydrogel, antibiotic, Rx may temporally stopped until
healing
• Pigmentation
Late-
• Hypo/ hyper -pigmentation
• Atrophy
• Alopecia
• Telangiectasia
• Cartilage and bone necrosis (now rare as electron beam used)
• Bone fracture
• Non healing ulcer.
• Secondary malignancy
Ca cervix radiotherapy complication
Complication of carcinoma cervix radiation:
Acute:
- Pruritus
- Desquamation
- Nausea
- Colitis
- Proctitis
- Cystitis
- Vaginitis
- Uterine perforation
- Vaginal laceration
Late:
- Stenosis
- Urethral stricture
- VVF
- RVF
- Intestinal obstruction/ perforation
- Femoral head necrosis (Dose below 50 yrs)
Complication in npc
COMPLICATIONS
Acute: mucositis, dermatitis, xerostomia.
Late: soft tissue fibrosis, trismus, xerostomia, hearing
loss, vasculopathy, osteoradionecrosis, temporal lobe necrosis, hypothyroidism, hypopituitarism (if included).
Lung RT complication
COMPLICATIONS
Acute RT complications include fatigue, esophagitis, der- matitis, and/or cough.
Subacute and late complications include pneumonitis, pericarditis, pulmonary fibrosis, bronchial or esophageal stricture, brachial plexopathy, rib fracture or intercostal nerve pain.
Radiation pneumonitis occurs ~6 weeks after RT. It pres- ents with cough, dyspnea, hypoxia, and fever. Treat symp- tomatic radiation pneumonitis with prednisone (
Breast RT complication
Complications of Treatment
• Skin changes including redness, dryness, fibrosis, and darkening, sore throat
• Inflammation of the lung with dry cough (radiation pneumonitis), 6 weeks after
treatment
—Lymphedema of the breast and the ipsilateral arm
• Ipsilateral chest wall rib fracture
• Second malignancy
• Cardiotoxicity
Complication of Radical RT:
1. Acute skin reaction:
a. Erythema
b. Dry desquamation
c. Wet desquamation
d. Psuedosclerodermatous panniculitis
2. Late cosmetic impairment: edema, fibrosis, telangiectasia, including risk of
breast reconstruction complication and/or cosmetic impairment
Other:
1. Upper extremity lymphedema
a. Elevation of the extremity
b. Compression treatment
c. Complex physiotherapy
2. Brachial plexopathy
3. Pneumonitis
4. Impaired shoulder joint mobility
5. Cardiac toxicity
6. Second malignancies: sarcoma, lung cancer, contralateral breast cancer
Complication of pelvic RT
Acute
Skin: erythema, hyperpigmentation, dry desquamation, moist desquamation
Urtethritis
Prostitis
Cystitis, myelosupression
Fatigue general weakness
Asthenia
Late
Skin fibrosis, telangiectasia
Long-term GI complications include change in bowel habits, rectal urgency, diarrhea, anastomotic stricture, and small bowel obstruction. Women are at risk for sterility, early menopause, and vaginal stenosis. Men should be counseled about infertility and given information about sperm banking.
