What is the definition of a pressure ulcer?
An area of localised damage to the skin and underlying tissue caused by pressure, shear or friction
What is a class one ulcer?
Non-blanchable erythema
What is a class two ulcer?
Partial thickness loss and superficial ulcer
What is a class three ulcer?
Full thickness loss + damage or necrosis
What is a class four ulcer?
Extensive destruction + tissue necrosis + muscle or bone damage
What increases the risk of developing an ulcer?
Acute illness, increased age, reduced consciousness, reduced mobility, reduced sensation
How can ulcers be prevented?
Supportive surface, regular skin inspection, keep moving, incontinence and moisture control, good nutrition and hydration
What increases the risk of abuse?
Cognitive impairment, functional dependency, low income, drug and alcohol use, low social support
