Describe seated ME for type 2 upper T1-4 thoracic
Pt seated
Student behind pt opposite PTP
Monitor spinous process and use other hand to flex/extend to barrier
Now monitor at PTP and place other hand on head to rotate and side bend
Pt tries to return to neutral against resistance
Describe seated ME for type 2 lower T5-12 thoracic
Pt seated with hand on side of PTP on back of neck and other hand grasping elbow
Student behind pt opposite PTP
Monitor spinous process and PTP
Use other hand to reach over arm grasping elbow to grab opposite bicep
Move pt i to flex/ext, rotate, side bend barriers
Pt trues to move to neutral against resistance
Descirbe HVLA Kirskville Crunch for thoracic
Pt supine with arms across chest, dysfunctional side arm superior
Student standing on side opposite Ptp
Roll pt towards self to place caudad thenar eminence on PTP
For flexion dys, could place thenar inferior to PTP
Flex upper extremities to place elbows over PTP
engage side bending barrier
Roll pt back and past midline
Upon pt exhalation, provide anterior to posterior thrust
Describe prone HVLA Texas twist for thoracic
Pt prone
Student opposite PTP
place thenar eminence of caudad hand on PTP with fingers cephalad
Place cephalad hand on opposite transverse with fingers caudad
Provide anterior force while simultaneously moving hands in direction fingers are pointing
At end of last exhalation, provide downward anterior thrust with twisting motion
Force is slightly greater on PTP
Describe supine HVLA over thigh for thoracic
Pt supine
Student places flexed knee on table so pt’s thoracic spine rests on thigh
Pt clasps hands behind head with elbows outward
Student passes hands through space of pt’s arms to encircle rib cage with fingers over posterolateral region of thoracic cage
After exhalation, pull chest downward onto thigh while adding cephalad traction on cage
Describe seated HVLA for thoracic
Student behind seated pt and places foot on side of PTP on table
Pt interlocks fingers and places hands behind neck
Place ipsilateral knee on PTP
Slide arms under pt axilla and through space of pt arms onto dorsal surface of pt wrists
Add flexion/extension for type 2
Lean pt back and upon exhalation, exert upward distracting force through axilla, using knee as fulcrum
Perform seated ME for type 1 thoracic
Pt seated with hand on side of PTP grasping back of neck while other hand grasps elbow
Student behind pt opposite PTP
Monitor apex of curve
Reach across front and pass under pt’s axilla to grasp opposite bicep
Induce side bending and rotation (no flexion/extension)
Pt moves to neutral against resistance
