  • Bruising (e.g. in a non-mobile baby).
  • Fractures: particularly metaphyseal, posterior rib fractures or multiple fractures at different stages of healing.
  • Torn frenulum: forcing a bottle into child’s mouth.

  • FTT.
  • STIs.

  • Emotional - not providing care/love expected from a parent/care giver e.g. shouting, threatening, manipulating, scape-boating, blaming, bullying, grooming, exploitation.

  • Neglect - malnourished, dirty clothes, poor dental hygiene, no access to medical care/prescriptions.

Are spiral fractures suspicious in a mobile toddler?

A
  • No as they can occur from rotating forces e.g. tibia spiral fracture from falling short on an extended leg whilst jumping. Known as ‘toddlers fractures’.
  • Always think about non-accidental injury if story is inconsistent with injuries.
  • Osteogenesis imperfecta can be prone to spiral fractures.

  • Technically it is statutory rape as legal age of consent is 16.
  • In reality you probably wouldn’t do anything if the girl has capacity and is in a consenting relationship. Red flags if there’s power imbalance or grooming.
