What is available in the community?
- Family, friends, community
- Voluntary sector
- GP
- Social prescribers (exercise, learn English etc)
- District nursing team
- Community therapy team
- Care navigators
- Social care (SW and formal carers)
- Community Mental Health teams
- Specialist physical community teams (eg HF, diabetes, rest)
- Community Palliative care (at home and hospice)
- Consultant input (geriatrician, psychiatry)
Types of accommodation available?
- own home (renting vs ownership)
- Sheltered accommodation
- Extra Care Sheltered accommodation
(both of these are most commonly rented through the council; Sheltered may only have staff onsite for a few hours; Extra has carers in 24/7) - Supported Living Scheme (tend to be for adults with a learning disability, similar to a Residential home but less support)
- Residential Home
(Council may have contract with the residential home, or can pay privately, carers 24/7, District nursing team needed for nursing interventions, traditionally not linked to a single GP practice, CQC registered) - Nursing Home
(privately run, council may pay if eligible, CQC registered, a lot of end of life care, has to be supported by a named GP practice with regular input) - NHS Continuing Care ward
(now disappearing, complex needs but not end of life)
Who pays for care?
not free at point of delivery in England (unlike NHS)
saving and land (not house) is >23k then you pay all.
between 14-23k the local authority will assess.
ongoing NHS care is free.
Rehab at home is free.
Reablement care is free for 6/52
Minor adaptations and equipment <1k is free.
CHC and Fast Track?
Continuing Healthcare ‘Fast Track’ if life expectancy <12weeks then care will be free (dont have to come into hospital to get it but often are sent home with it).
- meant to happen within 24hrs, gets people out of hospital.
MDT form.
If they live longer than 12 weeks then they are reassessed at that point.
What is available if not eligible for Fast Track?
CHC assessment for ongoing NHS funding.
Discharge to assess. Needs assessed in the community.
MDT “checklist” assessment is 15 pages or more. Full assessment can be 54 pages long!
