Advanced macrolide/ Doxycycline

amoxicillin OR Augmentin
CAP comorbidities
COPD, DM, liver or renal failure, CHF, alcoholism, cancer, asplenia
Outpatient w/ Suspected aspiration with infection
Augmentin or Clindamycin
Outpatient w/ Influenza with bacterial superinfection
Oral BL OR Respiratory FQ
Hospitalized general ward
Respiratory FQ
OR
Advanced ML + BL
Hospitalized (Intensive Care Unit)
P. aeruginosa but no BL allergy
Antipseudomonal BL + one of the following
1) Cipro or 2) Levofloxacin
3) (aminoglycoside + azithromycin)
4) antipseudomal FQ + aminoglycoside
Hospitalized (Intensive Care Unit)
No P. aeruginosa or BL allergy
BL + one of the following
azithromycin or respiratory FQ
Hospitalized (Intensive Care Unit)
P. aeuroginosa and BL allergy
Substitute aztreonam for the antipseudomonal BL
1) Ciprofloxacin, 2) levofloxacin
3) an aminoglycoside PLUS azithromycin
4) an antipseudomal FQ PLUS an aminoglycoside
Hospitalized (Intensive Care Unit)
No P. aeuroginosa and BL allergy
Respiratory FQ ± clindamycin
