Active Listening

Health Literacy
Level of health education and understanding a patient has acquired over time
Meaningful Access
Process of providing healthcare, education and resources; often starting from the moment a patient calls for a healthcare appointment or walks into the medical facility, and continuing throughout all interactions with healthcare environment.
Patient-Centered Care
Provides patients with information and options so they can make an informed decision
Patient Education
Provides high-quality health literacy and instructions relevant to patients care plan
Patient Provider
Healthcare professional delivering services to a patient (examples include: physician, hospital, nurse practitioner, physician assistant or other staff that can diagnose/prescribe healthcare treatments)
Release of Information (ROI)
Needs to be signed in order for patient information to be released to an external provider, caregiver or to anyone other than the patient
Right to Health
Basic human standard; provides access to the highest achievable health in an affordable, timely and appropriate manner; includes having basic needs met that impact the quality of one’s health
Self-Determination
A person taking action in their life to get the things they want and need
Situation, Background, Assessment and Recommendation (SBAR)
Standardized communication tool used by healthcare team members to brief one another and streamline patient care
Vaccination
Method of administering a small amount of a disease in order to gain immunity and protection and minimize effects and spread of a disease
Within Defined Limits (WDL)
Minimum and maximum amounts indicating normal patient lab resuslts
