Introduction of self
Explain purpose of visit - consent to discussion
Offer support person
Ask open question about how they feel/what they understand/what questions they have.
Break bad news or explain situation
Have silence, check understanding.
At the end - offer written information
Offer FU
Offer SW/MMH/compliant process
Outline plan of action to prevent it in the future

Do’s:
- Say sorry
- Explain clearly
- Remember about physical well being (consider PP care, VTE, contraception etc)
Don’ts:
- Don’t criticise colleagues
- Don’t blame patient
- Don’t be defensive
