what is intussusception

A

when the bowel invaginates or telescopes into itself

  • this results in a thickened bowel and narrowed lumen at the folded area, which can result in dilatation of the small bowel for example
  • causes a palpable mass in the abdomen and obstruction to the passage of faeces

presentation of intussusception

A

Severe, colicky abdominal pain
Pale, lethargic and unwell child
“Redcurrant jelly stool”
Right upper quadrant mass on palpation. This is described as “sausage-shaped”
Vomiting
Intestinal obstruction (constipation, abdominal distension)

why can a viral illness lead to intussusception

A

produces swelling of the lining of the intestine, which then slips into the intestine below

what causes the redcurrant jelly in intussusception

A

The trapped section of bowel may have its blood supply cut off, which causes ischaemia.
Mucosa is sensitive to ischaemia and responds by causing sloughing off into the gut.
This creates a “red currant jelly” stool, which is sloughed mucosa, blood, and mucus
