Heparin: Mechanism

A

Binds to, and enhances the activity of antithrombin III
- Neutralizes thrombin within seconds and inhibition of fibrin formation
- Inhibition of activated factor Xa
Prolongs clotting time in-vivo and in-vitro
- In-vitro prevents clotting in a test tube

Vitamin K antagonist
   - Inhibits formation of prothrombin and several clotting factors
       - Synthesis of these factors require vitamin K
absorbed rapidly from GI tract
Lowering of prothrombin occurs slowly
Extensively plasma protein bound
Narrow therapeutic index
Readily crosses membranes
Metabolized by hepatic cells
Delayed anticoagulant effect
   - Not useful in emergencies
   - Effect lasts for days after discontinuation
