Initial steps
A
- Ask the nurse the residents needs, abilities, and limitations
- Knock and identify yourself before entering residents room. Wait for permission
- Greet resident by preferred name
- Identify yourself by name and title
- Explain what you’ll be doing and encourage resident to help
- Gather supplies and check equipment
- Close curtains, drapes, doors. Cover resident only exposing body part you’ll be using
- Wash hands
- Wear gloves
- Use proper body mechanics. Raise bed (lower side rails)

Final steps
A
- Remove gloves and wash hands
- Be sure resident is comfortable and had proper body alignment
- Lower bed and put up rails if used
- Place call light and water on residents unaffected side
- Ask if resident needs anything else
- Thank the resident
- Remove supplies and clean equipment
- Open curtains,drapes and door
- Do a visual safety check
- Report unexpected findings to the nurse
- Document procedure
