Vitamin k antagonists — moa
Block reduction of vit k epoxide = a cofactor in the production of factors 2,7,9,10
Vitamin k antagonists - indication
Prophylaxis -
Dvt/pe/af clot/rheumatic disease/prosthetic valves
Vit k antagonist - adverse
Haemorrhage /
don’t give if HTN, pregnant, peptic ulcers cerebral thrombosis / peripheral artery occlusion
Heparin / LMWH- moa
Heparin activates antithrombin 3 = limits clotting by inactivating thrombin/factor X. Inhibits platelet aggregation. LMWH = similar but can be given less frequently and dosing more predictable
Heparin/LMWH - indication
(Clexane) - enoxaparin
Treating dvt and pe
Prophylaxis against post op dvt and pe
Heparin/LMWH- contraindication
Heparin no given to patients with HAEMOPHILLIA / THROMBOCYTOPENIA / PEPTIC ULCERS
advertse affects
- haemorrhage ( can stop infusion or give heparin antagonist e.g. protamine sulphate)
- thrombocytopenia
- hypersensitivity reactions
LMWH /heparin -region
LMWH - once daily sub cut
Heparin - iv infusion or sub cut 12 hrs
