ABCDE approach, call seniors early if the patient is sick.
if you don’t know the answer, say you don’t know, and explain how in that situation you would go about problem solving the answer - looking it up in the BNF,
Why do you want to do pads?
i told them i love kids and when i did internship in paeds i was told by my seniors that i am good with kids
What is difference in practice in uk and your country you graduated ?
highlighted that from as small a thing as using disposable tourniquets to having a specific stroke unit which doesn’t exists in my home country.
What do i expect to learn from this job
rn about paeds management and learn about anything n everything that comes my way in the process of serving my patients - I dont want to hold a tunnel vision n just focus on peds surgery n how to take out their appendix for example but to have a holistic approach to managing children as a whole
paeds SURGERY montegial fracture fixation and now almost 8 hours post op has pain and is on a back slab - what will u do and what is a montegeal fracture
i will Introduce myself , socilize a bit with the child , A-E approach , attach o2 , ecg , oximetry , bloods and give pain killers
child is not cooperating and is already topped up on morphine and you cannot give him more. But his pain is still bad what will u do
I will call my seniors, open the bandage and remove the back slab and see whats happening down there
DDx for such a pain
infection , bleeding
none of those - what is pain out of proportion caused by?
compartment syndrome
so what will you do now that you are sure it is compartment syndrome ?
management is immediate fasciotomy
since i know my limitations i will wait for my senior to come and do one
