Awareness of one’s own illness or situation

Circumstantial
Move from thought to thought that relate in some way but never get to the point; wandering from topic and never returning to original point.
Tangential
Illogical shifting between unrelated topics
Loose
Groupings of words (usually rhyming) based on similar sounding sounds, regardless of logical grouping.
Clang associations
Linear progression of thought without veering from subject at hand
Goal-directed/Logical
Quickly moving from one idea to another - often seen with mania.
Flight of ideas
Thoughts are interrupted; abrupt cessation of speech without explanation in the middle of a sentence.

Repetition of words, phrases, ideas

Randomly spoken words

Suicidal or homicidal ideation, preservations, obsessions, compulsions

False sensory perceptions; can be auditory, visual tactile, olfactory
Hallucinations
Feelings the outer environment feels real
Derealization
Sensation of unreality concerning oneself or parts of oneself
Depersonalization
Fixed, false beliefs firmly held in spite of contradictory evidence
Delusions
Name the delusion type – conspired against, spied on, harassed; others trying to cause harm
Persecutory delusion
Name the delusion type – patient has a physical defect
Somatic delusion
Name the delusion type – thinks spouse/love is being unfaithful
Jealous delusion
Name the delusion type – someone/people are in love with them

Name the delusion type – conviction that they have some great talent/insight or some great discovery; inflated sense of self-worth, power, wealth
