- O: Onset of pain
- P: Provocation. What makes the pain feel better or worse.
- Q: Quality. Describe pain:dull, sharp, crushing?
- R: Radiation. Does it hurt anywhere else?
- S: Severity. Rate the pain 1-10.
- T: Time. Does the pain come and go? How long does it stay for?
